Rehabilitation team members provide individual therapy programs with a focus on function. They work with the patient and their family to develop effective rehabilitation goals and discharge plans. The rehabilitation team includes the patient and their family, and, where applicable, a dietitian, nurse, physiotherapist, occupational therapist, doctors, speech and language pathologist, a social worker, rehabilitation assistant, and respiratory therapist.
Rehabilitation is the process of enabling an individual to reach an optimal mental, physical, and/or social functional level. The process is goal oriented and time limited. Rehabilitation addresses the needs of the individual and the environment in which he or she functions, whether at home, work, school, or socially.
